Heat olive oil in a large deep skillet or Dutch oven over medium heat. Add onion and cook for 4–5 minutes until soft and translucent.
Stir in garlic and cook for 30 seconds until fragrant.
Add ground beef, breaking it up with a spoon, and cook until browned. Drain excess fat if necessary.
Stir in tomato paste, oregano, basil, Worcestershire sauce, tomato sauce, and crushed tomatoes until well combined.
Add sugar, salt, and black pepper, stirring to combine.
Add macaroni and pour in beef broth. Stir to submerge the pasta, then bring to a gentle simmer.
Lower heat, cover, and cook for 12–15 minutes, stirring occasionally, until macaroni is tender and sauce has thickened.
Remove from heat and stir in cheddar and Parmesan cheeses until melted.
Stir in butter until fully incorporated. Taste and adjust seasoning if needed. Serve immediately.
Stir occasionally to prevent macaroni from sticking.
Use sharp cheddar for a more pronounced cheesy flavor.
Can be topped with extra Parmesan or fresh parsley for garnish.
